Required documents

For all applicants

1. Prescribed Application Form
2. Latest passport sized colored photographs signed by the applicant
3. Copy of CNIC
4. Banks statements of last year of all bank accounts
5. Copy of any of the latest utility bills or credit card bills
6. Proof of residence
7. Basic Fact sheet as provided by the Bank

For salaried individuals

1. Salary certificate specifying the name, CNIC No., date of joining, designation, salary details including net take home salary and status of employment (permanent, temporary, contract etc)
2. Copy of applicants salary slips for last three months
3. Last two years income or wealth tax returns or tax deduction certificate
4. Any other income detail

For self employed

1. Copy of registration of the relevant professional body
2. Copy of last two years income or wealth tax returns
3. Any other Income detail

For sole proprietors / partners of Partnership concern

1. Proof of business (last two years income or wealth tax returns)
2. Copy of registered partnership deed and acknowledgement Form-C
3. Last two years profit & loss statement and balance Sheet or audited accounts (where applicable)
4. Bank certificate stating that applicant is maintaining ‘business account’
5. Any other Income detail
